Consent to Upload and Processing

Before you upload, please read and confirm:

• IntelClaim is a self-help tool that helps you organize a medical-billing or insurance-denial case and create your own appeal materials. It is not medical, legal, or insurance advice, it is not a law firm, and it does not guarantee any outcome.
• The documents you upload (such as denial letters, Explanations of Benefits, provider bills, and insurer responses) may contain health and insurance information about you.
• You confirm that this information is about you, or that you are authorized to upload it and ask IntelClaim to process it for this self-help purpose.
• IntelClaim uses automated document reading (OCR) to pull key details from your documents and pre-fill a form that you review and correct. You are always in control of the final information.
• When Case Agent is enabled, you authorize IntelClaim to use a U.S.-based generative AI service provided through Amazon Bedrock under IntelClaim's AWS Business Associate Agreement to process the page text of the documents you upload, the structured facts and Evidence Ledger derived from those documents, and information you confirm, only to analyze your case, identify evidence gaps, prepare a strategy, and draft appeal materials. AI output may be incomplete or inaccurate, and you will review the materials before you use them. IntelClaim does not submit appeals or contact your insurer or provider.
• Your documents and case information are stored and processed by HIPAA-eligible service providers under a Business Associate Agreement, encrypted in transit and at rest.
• IntelClaim does not submit your appeal, send emails or faxes, or contact your insurer or provider for you. You decide when and how to submit.
• You can ask IntelClaim to delete your case or export your case information at any time, subject to the limits described in the Privacy Notice.
• You are responsible for reviewing everything IntelClaim generates before you use or submit it.

Mobile number and optional reminders

We use this number for one-time verification codes and security notices you request. These operational texts are separate from optional case reminders.

By checking this optional box, I agree to receive recurring case-reminder texts from IntelClaim. Message frequency varies. Message and data rates may apply. Reply STOP to opt out or HELP for help. Consent to reminder texts is not required to use IntelClaim. The reminder choice is separate from upload consent and may be left unchecked.
